Transition

Transition is when a child moves from home to nursery or school for the first time, or when they move from school to school or FE college. Children and Young people often need additional support at these times.

When is additional support provided for transition offered?

At these key times:

  • From home to nursery/school
  • From nursery to school
  • Between schools: Infant to Junior and Primary/Junior to Secondary, change of school within year
  • Secondary to Further Education
What does transition support look like?

We will support in the following ways:

  • Keeping in touch with the child/young person and their family regarding setting application and placement
  • Work with SENCo of settings, sharing relevant paperwork
  • Offer training in new setting
  • Gather pupil voice/all about me/ preparation for adulthood questionnaires
What additional support may be offered?

Where relevant we may:

  • Hold a transition review with current and new school
  • Organise transition visits to the new setting in addition to the city-wide transition day
  • Complete/update management plan
  • Carry out joint visits with the new Teacher of the Deaf when necessary
  • Invite to transition event (online or in person)
  • Prepare an experience book to prepare the child for their new staff and setting (nursery-reception)
  • Carry out assessments and share outcomes and targets with new setting
  • Liaise with health and social care
  • Prepare radio aid equipment
  • Complete environmental audit
  • Provide additional support in the first term in new setting
  • Attend EHCP reviews
Tips for parents from parents
  • Attend all meetings with the Teacher of the Deaf and staff from the current and new schools. Prepare any questions in advance.
  • Experience books for pre-schoolers starting school are useful for preparing children for their new staff and environment. Look at these with your child when you can to talk about the upcoming transition
  • Visit the new school with your child if you can and attend school open days
  • Look at the school website with your child
  • Communication was key. We have been regularly updated by phone and were kept informed of any changes
  • We felt included and so supported with the transition
  • We were involved at every stage and the outcome was brilliant for my daughter
  • The teacher has advised school to ensure that the provision is in place to meet my daughter’s needs

  • “It was good to meet older children who could answer my questions and make me feel better”
  • “It will help me for what I will do with my hearing aids. I need to remember my batteries. I feel more confident”

Preparing for adulthood looks to help beyond school, supporting young people as they move into adulthood, ensuring they enjoy living independently as adults.
